The George Maloney Industry Member of the Year Award
Nomination Form

This award was established to recognize the efforts of an individual industry member who has demonstrated exemplary support to SNA of Massachusetts, SNA members and their professional community. The award will be given annually to an individual who is engaged in the sale of products or services to members of SNA and who has made especially outstanding contributions to the advancement of the child nutrition profession. 

Nominations may be submitted by any SNA of MA member in good standing. Nominators may submit only one nomination for consideration. Additional nominations will not be considered. The current Executive Committee will determine final judging and selection of the awardees’, in the event of a tie; the Immediate Past President will break the tie. 

The recipient of this honor will be recognized during the Annual Conference with a plaque. They will also receive recognition on the SNA of Massachusetts Web site and in the What's Happening Blast. 

Nominations must be received by June 15th.    

Eligibility Criteria:

* Nominee must be employed by a current SNA Industry Member. 
Nominee must have maintained membership in SNA for the past three consecutive years.
* Current SNA Board members are not eligible for nomination.

Please provide the following information about the person you are nominating. Missing information may result in the nomination not being considered.

Please write a brief narrative to help further illustrate how this person has impacted SNA of Mass. and the SN industry. Some ideas to consider might include your own personal experience with the candidate along with some of the following points:

  • What contributions has this person made to SNA of Massachusetts? 
  • What actions has the candidate made to help promote child nutrition? 
  • Is the candidate involved with other groups/activities who promote child nutrition? 
  • Describe any career award or highlights.
  • Additional information that shows the candidate is deserving of this award.
